Congressional Medal of Honor License Plates may be issued at not cost for the first set of plate to any Illinois resident who has been awarded this medal from the U. S. Armed Forces.

A surviving spouse of a person awarded the Congressional Medal of Honor may obtain and/or retain these plates.

Fees

Random-Number Congressional Medal of Honor License Plates

  • Newly acquired vehicle/first-time issuance
    $95 - 1st set of plates (No registration fee + $95 title fee)
    $194 - 2nd set of plates ($95 title fee + $99 registration fee)
  • Currently titled vehicle/first-time issuance
    No Fee - 1st set of plates
    $99 - 2nd set of plates
  • Current plates expire within 90 days
    $29 - 1st set of plates ($29 replacement fee)
    $128 - 2nd set of plates ($99 registration fee + $29 replacement fee)
  • Current plates do not expire within 90 days
    $29 - 1st set of plates ($29 replacement fee)
    $29 - 2nd set of plates ($29 replacement fee)
  • Annual renewal
    No Fee - 1st set of plates
    $99 - 2nd set of plates

To Apply

  • One must provide a form from the federal government or a letter from the Department of Veterans' Affairs verifying that the applicant received the medal.
  • Applicants who do not have this information may contact:

  • The request can only be processed in Springfield. Apply in person or mail the proper documentation to:

Availability

  • Congressional Medal of Honor license plates are multi-year and may be displayed on passenger vehicles, trucks and vans weighing 8,000 pounds or less and sport utility vehicles.
  • The surviving spouse of a person awarded the Congressional Medal of Honor may obtain and/or retain these plates.
  • Not available as Vanity or Personalized
  • Not eligible for Circuit Breaker

Renewal Methods

  • The no fee Congressional of Medal of Honor license plates must be renewed in Springfield. Apply in person or mail the proper documentation to:

  • Only the second set of Congressional Medal of Honor license plates can be renewed online, by mail, touch-tone phone, or visit a Driver Services facility.
