About the Mattoon
Death Certificate Registers Index
The Mattoon Death Certificate Registers Index was
compiled by Amanda Standerfer, intern for the Illinois Regional
Archives Depository (IRAD) System at Eastern Illinois University in
Charleston. The database indexes nearly 3,600 death records that
were recorded by the local registrar in the City of Mattoon. The
City of Mattoon is located in Coles County. The information in the
database was extracted from the Mattoon Death Certificate Registers
(IRAD Accession 5/0077/01).
Each entry found in the index contains the
following categories of information: the name of the decedent; the
date of death; and the death certificate number. Additional remarks
may appear in the “Notes” category. Every attempt was made to
obtain accurate spellings of names. However, it was sometimes
difficult to decipher handwriting in some entries. Therefore, when
searching this index, we suggest that researchers check alternative
spellings of names if they do not find an entry for the name for
which they are searching.
Content of
Mattoon Death Certificate Registers
The death registers show the certificate number;
the date the certificate was filed; the name of the deceased; the
date of death; the cause of death; the name of the attending
physician; the interment number; the name of the person to whom the
permit was issued; and the date of return. Registers occasionally
include death certificates and permits from the local board of
health for transportation of a corpse.
Death certificates show the name, place of birth,
sex, color and age of the deceased; the number of years of residence
in Illinois; the date of death; the date and place of burial; the
name and address of the undertaker; the immediate cause of death;
the contributory cause of death; the duration of illness; and the
signature and address of the attending physician. Permits from the
local board of health for transportation of a corpse show the name
and address of the deceased; the time of death; the cause of death;
and the signature of the president or clerk of the board of health.

may be obtained by mail or telephone. Inquiries should be made
directly to the Illinois Regional Archives Depository (IRAD) at
Eastern Illinois University in Charleston. IRAD cannot accept

the records for themselves. The Illinois Regional Archives
Depository at Eastern Illinois University is open from 9:00 a.m. to
4:00 p.m., Monday through Friday. The depository is closed on all
state holidays and any day that the university library is closed. It
is always a good idea to call ahead before planning a visit.
